Sharepoint to Make Care for High-Risk Patients Less Risky

Project Summary

Northwell Health is a nonprofit integrated healthcare network that is New York State’s largest healthcare provider and private employer, with more than 74,000 employees in 2020

It was formerly called North Shore-Long Island Jewish Health System and renamed in 2015.

They had a desire to improve the performance in their preventative patient activity with a “Value based Gaps in Care” project initiative; which was to semi automate the identification of gaps in patient activity with Northwell’s doctors.   Currently this process was done via Excel exports, macros and manual reports.

The SharePoint initiative automated the data synchronization with SharePoint lists, where users will update / complete forms (tasks).  These tasks are updated back to the SQL database (Northwell’s system of record).

At the outset, Northwell established goals for integrating SharePoint and their system of record to include:

  • Exchange discrete data elements between systems to enable clinical decision support, analytics, and alerting
  • Transmit physician narratives for smooth transitions of care.
